AHCCCS compliance

The AHCCCS Daily Trip Report, filled out automatically.

AHCCCS requires one Daily Trip Report per member, per day, as supporting documentation for every NEMT claim. NEMT OS generates it from the trip itself: odometer readings, reason for visit, escort details, and electronic signatures, the moment a driver completes the trip.

How it works

From completed trip to signed report in one step.

1. Driver completes the trip

Pickup and drop-off odometer readings, reason for visit, escort name and relationship, and multiple-member indicator are captured in the driver app at the moment the trip finishes.

2. Signatures captured on the spot

Member and driver sign electronically on the driver's device. If the member is unable to sign, the app records who signed on their behalf: attendant, escort, guardian, parent, or provider.

3. Report generated automatically

NEMT OS fills the official AHCCCS Daily Trip Report PDF from that trip data. No one re-types pickup times, addresses, vehicle type, or mileage by hand.

4. Round trips combine into one report

AHCCCS requires one Daily Trip Report per member, per day. NEMT OS links a round trip's outbound and return legs and merges them into a single report: the 1st and 2nd trip sections of one document, not two separate reports.

5. Ready for the claim

Office staff can download the report immediately from the trip record, or regenerate it after correcting trip data, before it goes into the claim packet.

Why round trips matter

One member, one day, one report, even for a round trip.

The AHCCCS form is explicit: providers cannot bill two separate trips for the same member on the same day. Most systems dispatch a round trip as two independent trip records, which produces two separate reports, a documentation mismatch that can complicate a claim. NEMT OS links the outbound and return legs at creation, and once both are completed, combines them into a single AHCCCS Daily Trip Report: the outbound leg fills the 1st Pick-Up/Drop-Off section, the return leg fills the 2nd, and the Round Trip box is checked automatically.

AHCCCS questions

Direct answers about AHCCCS documentation.

What is the AHCCCS Daily Trip Report?

The AHCCCS Daily Trip Report is the supporting documentation Arizona Health Care Cost Containment System (AHCCCS) requires from non-emergency medical transportation providers for each member, per day, before a claim can be considered fully documented. It records pickup and drop-off details, odometer readings, reason for visit, escort information, and member and driver signatures.

Does AHCCCS require one report per member per day, even for round trips?

Yes. AHCCCS's Daily Trip Report form is explicit: one report per member, per day. A round trip's outbound and return legs must appear as the 1st and 2nd trip sections of a single report, not as two separate reports for the same member on the same day.

How does NEMT OS generate the AHCCCS Daily Trip Report automatically?

When a driver completes a trip in NEMT OS, the app captures pickup and drop-off odometer readings, reason for visit, escort name and relationship, and electronic member and driver signatures. NEMT OS then fills the official AHCCCS Daily Trip Report PDF from that data and makes it available for download immediately, from the driver app or the office dispatch view.

How does NEMT OS handle round trips on the AHCCCS report?

NEMT OS links a round trip's outbound and return legs at creation. When both legs are completed, it automatically combines them into a single AHCCCS Daily Trip Report: the outbound leg fills the 1st Pick-Up/Drop-Off section and the return leg fills the 2nd, with the Round Trip box checked, satisfying AHCCCS's one-report-per-member-per-day rule instead of generating two separate reports.

Can office staff regenerate a trip report after correcting trip data?

Yes. If trip details need correction after completion, office staff can regenerate the AHCCCS Daily Trip Report from current trip data before submitting it as claim documentation.
